David Michael Perkins, 51, of Cottage Grove, Ore., passed away Nov. 7, 2021. David was born to parents Fred and Sandra (Olson) Perkins on Oct. 21, 1970 in Cottage Grove.
He graduated from Elmira High School in 1988. David attended Oregon Poly Technical Institute and Lane Community College, receiving his associates degree at both schools.
